How to Make a Custom Brush With Photoshop
1: You open up Photoshop 7 (not sure about the past Photoshops).
2: File>New
3: Choose the custom Width and Height of your choice but it must be transparent content.
Keep in mind that it will be the size of the brush unless you choose not to draw all over the space. For example: As shown here.
4: With Tablet pen or Brush or Pen Tool or whatever tool you wanna use to draw your custom brush, just draw.
or
4: If you're not drawing and just wanna render something out of an image to use as a brush, I would suggest the Polygonal Lasso tool 'cause I think it makes life easier.
For Those Drawing Their Own Brush
5: Once you're done drawing whatever you want your custom brush to be, edit it or clean it up a little by doing fancy things to it and/or Crop it to keep close to the size of the brush rather than have extra space.
For Those Rendering an Object from an Image to Use as a Brush
5: Open whatever file you wanna render from (for this, I'll choose something a bit difficult). Crop whatever you want to make into a brush.
6: Click on Polygonal Lasso tool.
7: Click around the shape of the object to select the spaces you don't want. Make sure you connect the first anchor with the last to get that Select look.
Right-click and click on Select Inverse to select the space around the object.
8: Magic Eraser and Erase the selected space.
9: Deselect and begin cleaning up the render by Erasing spaces you don't need. Crop out the extra canvas space.
10: Then go to Image>Adjustments>Brightness/Contrast.
Turn down the Brightness 'cause brushes pretty much look better in black (my example is shoddy).

6: Once done with all that, click Edit and then go to Define Brush.
It will then ask you to name it. Name it.
7: To use and/or see your custom brush, go to the tab on the top right corner of Photoshop or look for the Brushes tab. Click it, scroll down through to the bottom of the brush palette and you'll see your newly made brush.

Saving a Brush Set
8: But if you want to make more to save as a set, you'll have to clear out the brush palette (hopefully you have your downloaded brushes and default brushes saved on your hard drive as backup) by deleting the other brushes. To do that, go to the Brushes tab again.
9: Starting from the top, have the mouse cursor on the first brush, hold down the Alt key until you see the mouse cursor turn into a pair of scissors, and click to your heart's content until you reach your custom brush (which hopefully you won't accidentally delete).
10: Then, drawing another brush (same steps you used for drawing the first brush), you Define Brush and name it like before. Then click on the Brushes tab, click the little black arrow next to "Brushes" in the tab, and click on Save Brushes...
11: Name the file. And you're done.
Don't mind my crappy rendering skills of the windows, my mouse died out so I had to use my tablet.
