Well, from what i've heard where Bush was (can remember where exactly) they view the foot or shoe as the dirtest part of the body. So its actually a pretty big insult to have a shoe thrown at you in that part of the world.

He's used to it, apparently. Some polititian-y American guy on the news said it was a good way of him showing his experience, or something.
